Chapter 18 guidelines
Symptoms, signs and abnormal clinical and laboratory findings, not elsewhere classified
R00-R99
Covers symptoms, signs, and abnormal clinical/laboratory findings that are not otherwise classified to a more definitive diagnosis elsewhere.
Key coding points
- •Codes from this chapter are appropriate when a definitive diagnosis has not yet been established, or when the sign/symptom is being reported on its own because no related definitive diagnosis was confirmed for that encounter.
- •If a definitive diagnosis is established, it should generally be coded instead of, or in addition to, the associated symptom, depending on payer and documentation guidance.
